Hints for completion, on-line.
When you open a form, the first fill-in area will have a darker gray
highlight, just begin typing.
Use the TAB key OR arrow keys to advance from field to field.
You may click the space bar OR the mouse button to insert
or remove an "x" in the check boxes.
You may wish to view your document at 100% in order to see more clearly.
Each fill-in area has a limited amount of space, you may need to attach
